Hello, I am Jan Rookard, Northeast Ohio I-CARE Coordinator. I take great pride in the mission of seeking out and locating resources to assist you in staying independent in your home.

I have 14 plus years in the social work field helping elders, adults with disabilities and family systems that require help in various ways. If you have any questions about how I-CARE can assist you or a family member, please do not hesitate to contact me. I am available to give you referrals, information on upcoming senior events or just a friendly conversation.

As the population of seniors grow so do their needs. The available “system of services” can be confusing and overwhelming. If you need help navigating this system so that you can remain independent give me a call because I-CARE!
